Lindsey Maloney
DNP, APRN, FNP-C
Founder/Chief Medical Officer
Lindsey Maloney is a Litchfield county native and began her healthcare journey as a CNA in the nursing home setting while pursuing a bachelor's degree in nursing at Western Connecticut State University. She practiced as a registered nurse in the ER, ICU, and home care settings for 6 years. Two years into nursing, she decided to pursue her doctorate in nursing practice at Quinnipiac University. Her early nurse practitioner career was spent in a community health center providing acute and chronic care to high-risk patients with multiple complex health conditions complicated by socioeconomic barriers. In 2020, in response to a rising number of elderly and disabled individuals seeking primary care in the home, she developed Nutmeg Primary Care (previously Mobile Care Partners of Connecticut).
Since establishing, the practice has grown exponentially, continuing to provide in-home primary care to disabled adults in their homes in addition to an outpatient family practice in Kent. In 2022, she became a partner at Preferred Primary, PLLC in New Milford. She was inducted into Connecticut Magazine's 40 Under 40 class of 2022 and honored as Connecticut’s APRN of the year for 2023. In her free time, Lindsey enjoys spending time with her family and friends, snuggling with her dog Cloud, stargazing, bird watching, and traveling.

Siena Giordano
DNP, APRN, FNP-C
Home Primary Care Provider
Siena Giordano is a Board Certified Family Nurse Practitioner. She graduated with her Bachelors in nursing from Quinnipiac University. After working as a nurse, she developed a growing desire to help individuals in a more direct and longitudinal capacity which brought her back to Quinnipiac to get her Doctor of Nursing Practice.
Siena joined Nutmeg Primary Care two years ago. She is honored to be able to provide primary care services to patients with complex medical conditions in the comfort of their home; a very personal role that she takes to heart. She enjoys developing relationships with patients and families and empowering them on their journey of health and wellbeing.
Siena is grateful to be able to serve patients in Litchfield County where she was born and raised and currently resides with her husband and two children.

Judilynn Ferlow
MSN, APRN, FNP-C
Family Nurse Practitioner
Judilynn Ferlow graduated from Carson-Newman University in Tennessee earning her Bachelor of Science in Nursing from Southern New Hampshire University and her Associate’s degree in nursing from Naugatuck Community College in Connecticut. Judilynn worked most of her career in pediatrics. In addition to nursing, Judilynn is a 13-year EMT and officer with New Milford Ambulance.
Judilynn hails from Allentown, PA but has spent a good part of her life in Connecticut. She has been married to her husband, James, for over 30 years and has three grown children – Cody, Nicholas, and Michaela. James is a 42-year firefighter and environmental scientist. She has been involved in many different groups in New Milford, including being team moms for her children’s teams, a longtime band parent with the high school band, a past member of the Women’s Club, a longtime girl scout leader and Clatter Valley Girl Scout camp leader and Assistant Director as well as a boy scout mom and Clatter Valley Cub Scout Camp leader and Director.
Judilynn likes to travel and hike, and spends a good deal of time with her family at their camps in Eagle Bay, NY in the Adirondacks. Judilynn and Jim have two crazy black labs named Huckleberry Finn and Mylie Mae, a 17-year-old orange cat named Chester Copperpot (Copper for short) and a busy life between family, friends, Nutmeg Primary Care, the firehouse, and the ambulance corp.

I’ve been with Lindsey since almost the beginning, the original Chaos Coordinator, I’m thrilled to have been able to assist Lindsey in this amazing journey. Nutmeg Primary Care believes in old fashioned medicine, where we partner with patients in all aspects of their health care needs. I’m proud to work alongside this group of amazing women.

Hi, it's me, Vicki, the Front End Coordinator, the face behind the calming voice when you call. During work hours, I enjoy helping patients navigate through insurance questions and ensuring easy scheduling for all of our patients. I find it important to treat every individual the way I would like to be treated even when you don't feel well. Outside of work hours, I can be found writing, gardening or trying to repurpose unloved items amongst all of the chaos life brings.
Hi, I’m Alexis, a proud Certified Medical Assistant and your go-to Chaos Coordinator, working closely with Lindsey and Siena. I enjoy juggling family life on my small farm in Sharon with my husband, two energetic sons, and a pack of crazy dogs! With over 15 years in healthcare under my belt and nursing school in progress, I balance soccer fields and taekwondo meets while soaking up the outdoors—rain or shine!
Hi my name is Cecelia and I am a mom of 5 children. When I am not working with Judilynn, I am enjoying quality time with my children and husband. I enjoy cooking and baking with my children on the weekends. I’m currently going to school for Criminal Justice and have 2 more years to go until I graduate.
Hello there! I'm Jennifer. I am not only a wife and mother, I am also a Medical Assistant with 30 years of hands-on experience. I work side by side with the providers. I have a passion for collecting unique items and tending to my plants and flowers. Excited to share the journey caring for you!
