Lindsey Maloney
DNP, APRN, FNP-C
Founder/Chief Medical Officer
Lindsey Maloney is a Litchfield county native and began her healthcare journey as a CNA in the nursing home setting while pursuing a bachelor's degree in nursing at Western Connecticut State University. She practiced as a registered nurse in the ER, ICU, and home care settings for 6 years. Two years into nursing, she decided to pursue her doctorate in nursing practice at Quinnipiac University. Her early nurse practitioner career was spent in a community health center providing acute and chronic care to high-risk patients with multiple complex health conditions complicated by socioeconomic barriers. In 2020, in response to a rising number of elderly and disabled individuals seeking primary care in the home, she developed Nutmeg Primary Care (previously Mobile Care Partners of Connecticut).
Since establishing, the practice has grown exponentially, continuing to provide in-home primary care to disabled adults in their homes in addition to an outpatient family practice in Kent. In 2022, she became a partner at Preferred Primary, PLLC in New Milford. She was inducted into Connecticut Magazine's 40 Under 40 class of 2022 and honored as Connecticut’s APRN of the year for 2023. In her free time, Lindsey enjoys spending time with her family and friends, snuggling with her dog Cloud, stargazing, bird watching, and traveling.
Siena Giordano
DNP, APRN, FNP-C
Home Primary Care Provider
Siena Giordano is a Board Certified Family Nurse Practitioner. She graduated with her Bachelors in nursing from Quinnipiac University. After working as a nurse, she developed a growing desire to help individuals in a more direct and longitudinal capacity which brought her back to Quinnipiac to get her Doctor of Nursing Practice.
Siena joined Nutmeg Primary Care two years ago. She is honored to be able to provide primary care services to patients with complex medical conditions in the comfort of their home; a very personal role that she takes to heart. She enjoys developing relationships with patients and families and empowering them on their journey of health and wellbeing.
Siena is grateful to be able to serve patients in Litchfield County where she was born and raised and currently resides with her husband and two children.

Judilynn Ferlow

Family Nurse Practitioner
Judilynn Ferlow graduated from Carson-Newman University in Tennessee earning her Bachelor of Science in Nursing from Southern New Hampshire University and her Associate’s degree in nursing from Naugatuck Community College in Connecticut. Judilynn worked most of her career in pediatrics. In addition to nursing, Judilynn is a 13-year EMT and officer with New Milford Ambulance.
Judilynn hails from Allentown, PA but has spent a good part of her life in Connecticut. She has been married to her husband, James, for over 30 years and has three grown children – Cody, Nicholas, and Michaela. James is a 42-year firefighter and environmental scientist. She has been involved in many different groups in New Milford, including being team moms for her children’s teams, a longtime band parent with the high school band, a past member of the Women’s Club, a longtime girl scout leader and Clatter Valley Girl Scout camp leader and Assistant Director as well as a boy scout mom and Clatter Valley Cub Scout Camp leader and Director.
Judilynn likes to travel and hike, and spends a good deal of time with her family at their camps in Eagle Bay, NY in the Adirondacks. Judilynn and Jim have two crazy black labs named Huckleberry Finn and Mylie Mae, a 17-year-old orange cat named Chester Copperpot (Copper for short) and a busy life between family, friends, Nutmeg Primary Care, the firehouse, and the ambulance corp.
Michele Gervase
DNP, APRN, FNP-CC
Family Nurse Practitioner
Michele Gervase is a board-certified Family Nurse Practitioner who earned her Doctor of Nursing Practice (DNP) degree from Sacred Heart University. A native of New York, Michele began her nursing career on Long Island, where she amassed over 13 years of clinical experience, primarily in cardiac surgery and heart failure. Her expertise was honed in the cardiothoracic intensive care unit, with additional experience in trauma, medical-surgical, and neurosurgical ICUs.
Following her frontline work during the COVID-19 pandemic in New York, Michele
relocated to Connecticut four years ago to pursue her doctoral studies. Her clinical
interests include diabetes management, cholesterol control, and lifestyle medicine. She is deeply passionate about empowering patients to reclaim their health and is currently working towards developing a comprehensive lifestyle medicine program aimed at promoting long-term wellness through sustainable lifestyle changes.
Outside of her professional life, Michele enjoys staying current with emerging medical research, traveling, spending time in nature, and relaxing with her spirited 13-year-old dachshund, Dexter. An accomplished home cook, she is especially known for her skill in crafting artisanal sourdough bread.​

Hi there, I'm Faith aka "The Mutha" and Practice Administrator. I have been with Lindsey since the very start, and I take great pride in all we've achieved together. The original chaos coordinator, I now work diligently behind the scenes to ensure everything runs smoothly.

My name is Brookelynn, and I’m proud to be the receptionist at Nutmeg Primary Care. I have 2 cats: Lily & Bella. When I'm not working I love to hike or go on sunset drives. I’m also pursuing my education in medical assisting. I love being the first friendly face patients see. My goal is to help everyone feel welcomed and at ease when they arrive.
Hi, I’m Alexis, a proud Certified Medical Assistant and your go-to Chaos Coordinator, working closely with Lindsey and Siena. I enjoy juggling family life on my small farm in Sharon with my husband, two energetic sons, and a pack of crazy dogs! With over 15 years in healthcare under my belt and nursing school in progress, I balance soccer fields and taekwondo meets while soaking up the outdoors—rain or shine!
Hi my name is Cecelia and I am a dedicated mom of 4 who balances my professional life working with Lindsey Maloney and my love for family time. When I'm not working, I enjoy being outdoors with my children, as well as cooking, baking, and exploring new foods.
Hello there! I'm Jennifer. I am not only a wife and mother, I am also a Medical Assistant with 30 years of hands-on experience. I work side by side with the providers. I have a passion for collecting unique items and tending to my plants and flowers. Excited to share the journey caring for you!

Hi my name is Virginia. I have been a Certified Nursing Assistant for five years and a Medical Assistant for almost four years, working alongside Michele. I am a mom to a beautiful girl. When I am not working I enjoy line dancing, hanging out with my family and friends. I love cooking and baking especially during the fall. Christmas is my favorite holiday and my favorite animal are penguins. I love working in the healthcare field and giving back to the community.
Hey, it's me Vicki, aka the "insurance guru" of the office, serving the practice with prior authorizations and billing support. Outside of work I value the most of my time spent with family. I love writing, gardening, hiking and playing tennis.
