We understand that life can be busy and your time is valuable. To accommodate your individualized needs, we offer the flexibility of both in-office visits and HIPAA-secure telemedicine options. Please note that for telemedicine appointments, you must be located within Connecticut per state law.

​Age Appropriate Vaccinations
We offer age-appropriate vaccinations for both adults and children at no cost for most payers. To learn more about your eligibility and to receive any appropriate vaccinations, please speak with one of our team members during your next visit.
Sick & Acute Care
We understand that sickness is never planned and often occurs at the most inconvenient times. That's why we offer acute care services when you need them most, striving to provide same-day appointments whenever possible.
Our skilled team is equipped to collect and interpret rapid/point of care testing for conditions such as urinary tract infections, respiratory viruses (including RSV, influenza, and COVID-19), streptococcal pharyngitis, pregnancy, and anemia. Additionally, if ordered by your provider, we can perform further diagnostic testing, administer in-office treatments, and assist in transferring you to a higher level of care if necessary.
To ensure continued access to care when our office is closed, we collaborate with Yale New Haven Health to provide you with easy access to urgent care services that cannot wait until the office is open.

Collaborative Care & Referrals
Our Home-Based Program delivers high quality primary care directly to the doorstep of qualifying adults in Litchfield County. Our program is tailored to provide you with the individualized, comprehensive primary care you need by eliminating the barriers that hinder your access to care. Through collaborative efforts with other healthcare providers, we coordinate the delivery of ancillary services as well as in-home imaging and blood draws.
We provide homebound services to individuals residing in assisted living facilities within our service area. Assisted living residents or directors interested in our services should contact us.
We understand that sometimes your care needs extend beyond the scope of primary care. That's why we collaborate with healthcare professionals across specialty and ancillary services to ensure you receive comprehensive, collaborative, multidisciplinary care.
